CHC Membership

Membership Overview
A CHC Membership allows one person to access to our programs as long as they are between the ages of 6-17. Membership does not expire.

Members 6-17 years of age and may register for programs and services offered by CHC for their age group. A parent or guardian’s signature is required.
Scoreboard
By filling out the membership enrollment form you will gain access to our Scoreboard system which will allow you to browse and register for programs, complete surveys, and redeem points for prizes in the Scoreboard store!
Cost
Membership and all of CHC's programs are FREE of charge.
Newsletter
Membership enrollment also gives you access to our quarterly newsletter that highlights specific CHC programs, current research, and gives you the first look at upcoming programs and events!
Become a Member

Fill out the Membership Form
To become a member, fill out our membership enrollment form.
Receive Membership Confirmation from Staff

You will be contacted by a CHC staff member within 3 business days confirming your membership.
Create a Scoreboard Account
03
Our CHC staff will help walk you through the steps to complete a Scoreboard account. This account will give you access to register for programs, view programming schedules, complete surveys, and earn points!
